BCBS keeps denying prior auth for my MRI even after my neurologist sent a peer-to-peer request. What am I missing?

So ive been trying to get an MRI authorized for about 6 weeks now and im losing my mind. Started having episodes of right-side weakness and visual disturbances in mid April. My PCP referred me to a neurologist who saw me within 8 days (small miracle honestly) and she ordered a brain MRI with and without contrast to rule out MS and a few other things.

BCBS PPO denied the initial request saying it didnt meet "medical necessity criteria" and asked for a peer-to-peer review. My neuro did the call, told me afterward she felt good about it, expected approval. A week later, denied again. Same form letter. Now they're saying we need to do 6 weeks of "conservative management" first, which apparently means PT and oral steroids. For symptoms my neuro literally said could be a brain lesion.

What am i actually supposed to do here? My neuro's office said they can do another peer-to-peer but is that just going to be the same outcome? Im in North Carolina if that matters. Do i file a formal appeal myself? Go to the state DOI? I cant exactly wait around for conservative management when nobody knows what is actually wrong yet.
