Finally won my appeal after 8 months - here's what actually worked
Long post but i hope this helps someone going through the same thing. My insurance denied a surgery my orthopedic surgeon said i needed for a torn meniscus. Said it was "not medically necessary" even though i could barely walk and had failed 6 months of physical therapy before the recommendation.
I appealed twice internally and got denied both times with what were basically form letters. The second denial letter was actually word for word the same as the first one, which made me think they didn't even read my appeal. I was so frustrated i almost just gave up and figured out how to pay out of pocket.
What finally worked was requesting an external review through my state insurance commissioner. I live in Ohio and apparently they have a solid external review process. Got an independent physician to review my case and they overturned the denial within 30 days. Insurance had to cover it.
The thing that made the difference was my doctor's office writing a super detailed letter of medical necessity that addressed every single point in the denial letter specifically. Not a generic letter - a point-by-point rebuttal with citations. If your internal appeals have been denied, look into external review in your state. It exists for exactly this situation and most people don't know about it.
