Finally won my appeal after 3 denials - here's what actually worked
I've been fighting my health insurance for 8 months over a surgery they called "not medically necessary." We're talking $34,000 out of pocket if I lose. I just got the final determination back and they're covering it. I'm still kind of shaking.
First two appeals I wrote myself and got denied both times with the same boilerplate language. Third time I did things differently. I got my doctor to write a very specific letter citing clinical guidelines (she referenced the exact CPT code and pulled actual peer-reviewed studies). I also requested their internal review criteria under ERISA and found their denial didn't actually match their own coverage guidelines. I included a one-page summary at the front laying out exactly why their denial was inconsistent with their own policy language.
If you're in the middle of a denial right now - don't give up. Request the entire claim file including the denial rationale. Ask for the name and credentials of whoever reviewed your claim. Make them justify every single decision. It is exhausting but it can work.
